My save is just before the end of the game, last codex to find to get codex and all collectibles achievements. Run to the judgement house but DON'T ENTER or you'll have to start a new game or reload the save. When you're at about 18m from the door look on the left side behind the two trashes, here is the last codex on the wall. Revel it, take the codex and the 2 achievements are yours. Now you can enter and complete the game. Note that its my 2nd playthrough so I didn't help Julie, Ingrid etc. so you can get these achievements too..
