I had the same issue using the rehash/resign via horizon but had to go change the device and console id to all zero's to get it to work.....

Mine does it as well.. Slim 4GB with 320GB HDD added to it.. Seems to do it 1 in 5 shut downs with disc inside, I found if I hit the power button...
