Every time I plug it into my xbox it says load faild I save rehash n resign with horizon whats wrong with it

 

Working fine for me.

 

Tested works fine......but some issues.
Health doesn't increase.
Strength and Magic get reset after a quest.
Can't break damage Limits(Want to deal 999k)Max I am getting is 30kish.
Something I am doing wrong?
