Sony have confirmed that there are issues with some of the PS4 consoles disc drives are having issues with ejecting. Its not yet known if its a specific line of drives, but it is known its not an isolated issue. Sony are now asking anyone that has had an issue with their drives ejecting to please answer some questions on their support forum: What title were you trying to eject? Were you installing any game data at the time? Did you see any game or system notifications beforehand (Ready to install notifications, error messages, etc.)? What troubleshooting steps did you attempt? So far there are a massive 20 pages on the support thread, so hopefully Sony will get to the bottom of the issue sooner rather than later.
