Drop the Trainer loader AND trainer.xex into the game folder with it's default.xex
Launch the Trainer Loader
Press start+back whilist in game

But I do indeed think it's +3 instead of +21.

If a trainer option works across multiple modes within a game, it's still +1 for that option.

Like, if you would have an option to freeze a timer, it doesn't matter it works across different modes. It's +1 option.

But alas, enough bitching. ;-) Excellent trainer for an excellent game! Thanks!
