How to mod Watch Dogs?

    I saw a video on youtube using a trainer to get unlimited ammo etc and modded game saves. I want to learn how to change the models in the game, weapons etc. Can anyone help me?

    TeamXPG made a +25 trainer for watch dogs, that includes unlimited ammo, inf health, inf money etc. I am unsure what it is you want to mod. The models? weapons? what is it you want to do?
